I don't have birth certificate issued by municipal authority though i have one issued by the hospital, but now as I have to apply for a fresh passport, I need to have municipal issued birth certificate. So how to apply for it ??How to apply for a new birth certificate ?

You can also make an affidavit, mentioning your date of birth etc, in a court.
You have to go to a Notary , who sit in the premises of any court and he will arrange everything at a fee.
You can produce this affidavit in place of birth certificate. It is valid.
