A recording gives instructions for submitting a written application by mail or ordering with a credit card. It may take 7-21 business days to process your request.



You can download an application at their website and mail it with a notarized signature, attach a check or money order payable to the 'Iowa Department of Public Health,'and a clear photocopy of government-issued photo id. The fee of $15.00 for the record search and includes one copy. Submit this to:
