How do I obtain my birth certificate from Taiwan?

Hello all! I was born in Taipei, Taiwan but am now a US citizen residing in NY. I need to obtain a copy of my birth certificate, which my parents have misplaced. Is there an office or website in Taiwan that I may contact to get a replacement copy? Thanks in advance for your help!How do I obtain my birth certificate from Taiwan?
Send a SASE (Self-Addressed Stamped Envelope) along with your proof of purchase and UPC code label to Customer Service.
Unless you know someone there who can make calls and find out which office could help you, the only thing I can think of is to use a service like the one listed below.



Just found some info for the city gov't of Taipei. Cannot find a dept that says it covers questions on birth certificates, but many city dep'ts have a contact email. You would probably need to contact the city hall where you were born to find the proper information.
You need to have your parents go to the gov't office in the place you were born to get a copy. They may need to bring the household registration with them.
Hi Nancy, I have just been to some sites, and they seem to point to one single direction. You need to go to the Household Registration Office (戶政事務所) where you were registered at the time of your birth. You may also apply for it at the hospital of your birth, but I would advise that you get it at the Household Registration Office. Some things I learned during my research:



1.) One of the respondents at the Chinese Y!A site advised to get multiple copies for possible future use. If you apply at the HRO, what you'll get are sealed copies of the original.

2.) It is possible to request a proxy to help you in matters, but this person needs to sign some papers I don't know what.



Today is Saturday and as such government offices are closed. Come Monday, I may be able to give you additional details. In the meantime, it is imperative that you know where you were registered at the time of your birth (戶籍). If possible, ask your parents to dig up the Certificate for Household Registration (戶口名簿), it must have your name on it. You will be able to trace your civil history there. You may also take a look at the back of your Taiwan National ID Card, if you still have it. The info should also be there.

Nancy, here's my latest update: I have just confirmed with the Taoyuan HRO that you need to apply for your Birth Certifcate at the HRO (戶政事務所) where your residence was registered at the time of your birth. So say for example, at the time of your birth, your parents registered your residence at the Shilin HRO (士林戶政事務所), you therefore have to go there to apply for your Birth Certificate. Every municipality has its own HRO. If you personally cannot be there for the reissue application, then any one of your parents or siblings will do. They will have to bring with them their National ID Card and personal seal/chop (印章). This is the most hassle-free way (a proxy must have an authorization letter, and this is an added problem in the processing of papers.) Sorry, but there is no English version of the Birth Certificate, as I stated last Saturday that there is (I will delete that part of the entry.) You will need to have it translated, and then notarized at a Taiwan District Court (地方法院). That's about it.
