How does someone not related to you get on a birth certificate?

Well, my friend's birth certificate has her apparent "adopted mom" on her birth certificate. She doesn't know why because she isn't her real mother as far as she knows.

So how could she get on the birth certificate?How does someone not related to you get on a birth certificate?
It's routine in an adoption for a new birth certificate to be issued with the names of the adoptive parents.
When you are legally adopted you get a new birth certificate with your adoptive parents names on it.
