What are the requirements to add a parent to a childs birth certificate?

What requirements would the potential parent need to be added to a birth certificate? If there is no father listed, would he need to take part in a paternity test to establish this? Can this person just add his name to the birth certificate by just calling the Vitals dept? Wouldn't the parent listed on the certificate have to be notified and sign off on it to make it legal?What are the requirements to add a parent to a childs birth certificate?
