Birth certificates are issued by the provinces, and each province has a different method of issuing birth certificates. As a result, there is no such thing as a "Canadian long form birth certificate", as it would be a "Ontario birth certificate" or a "Saskatchewan birth certificate", etc. Not every province offers a long and a short version.
As an example of something that might be on a longer version in some provinces, information like the names of parents' may be included.
