Since the State, County, City, Province, etc. government has your actual original birth record, what you had was a certified copy of your birth record, known as a certified Birth Certificate or Certification of Birth. To replace yours, simply google Ohio birth certificate or Ontario birth certificate, etc. and you'll find the office that issues birth certificates. Follow their instructions and you'll get another one.

I've lost mine before, you just go to the births deaths and mariages office (usualy local town hall) and get a duplicate which costs about 拢15

Yes you are still considered a citizen of the nation. You can still replace the old one. Replacement birth certificate could help you regarding this matter.
