I was raised from birth by my aunt and uncle in Prentiss, Mississippi and all of my records have their surname listed as my last name. I am having some difficulties now and need a corrected birth certificate with their names on it. Call the Vital Statistics Office for the State you were born in to find out what you need to do to have this birth certificate corrected. If your aunt and uncle adopted you when you were younger then they have to issue you a new birth certificate that shows the corrected name.
My mom had to do this with her home country of Panama when it was discovered that her mother's name was spelled wrong on her birth certificate. She got help from a lawyer to do that because of some business she has going on.

You actually have to go in person to the hospital where you were born and request it or call and ask them to explain the steps you need to take. other than this your parents themselves would need to do it for you and in person. hope this helps.How do I get a corrected birth certificate?
You cannot change a birth certificate without a court order. And you cannot just put their names on it either, they would have to adopt you - and yes, adults can be adopted. You could also do a legal name change, but that will not change your birth certificate. Talk to an attorney in or near Prentiss.
Also, contrary to some others' answers, birth certificates here in Mississippi are NOT in the courthouses. They are housed at the state's Vital Records Department in Jackson. Additionally, the hospital only has its own records of the forms they send to Vital Records. The problem is not with your birth certificate, which is the legal record, showing to whom you were actually born. The issue is the later records that showed an alternate name, with no legal authority behind that. The only way in which a birth certificate is legally amended to show other parents, is by adoption. As others mention, you do also have the option to legally change your name now to the one you have used, but I also doubt that would extend to altering the original birth record.
I believe your aunt and uncle have to adapt you legally in order for you to use their surname officially.
You call the court house in the county where you were born. Many court houses have web sites and you can fax in a request. But all court houses have a started form in which to hand in directly (some you can mail or fax) to fix or re-order your birth certificate.
I had to order a new one not too long ago and the process isn't that hard.
However if your birth name is not the same as the name you are using now, then you need to change your name legally and get the certified document from that. I have also done this. Both can be taken care of at the court house.
